Manager: How to Use the 30/60/90 Day Review Form

The first 90 days in a new role are critical to long-term success. A well-structured 30/60/90-day review ensures that employees start strong, with clear expectations, a solid training plan, and regular progress check-ins.
The review form is a tool for helping managers track job duties, competencies, and key milestones. It's designed to support structured onboarding and documentation, but it's not a replacement for cultural onboarding, team introductions, or compliance paperwork.

- Create a training plan based on job duties and tools (software, hardware, etc.).
- Ensure employees are progressing through the onboarding milestones.
- Identify gaps in training or support early on.
- Document progress and conversations over time.
✅ Where to find the Form
You can score your employee’s review as a manager in your dashboard under your direct reports > 30/60/90 Day Review.
🔧 Before the Employee Starts
- Review your department’s process for assigning the form and adding milestones.
- Talk with your system administrator if you need to update or customize the form.
- Double-check that the milestones listed match the expectations for this role.
- Outline your expectations for each phase of onboarding (see chart below).
📝 Using the Review Form
Each milestone on the form includes a comment section. Use it to:
- Document relevant notes or expectations in advance
- Add feedback during check-ins.
- Provide context for training decisions.
💬 Tip: Comments are automatically saved as you type, so your progress will not be lost even if you exit the page.
Once a milestone is complete:
Click "Yes" to confirm that the employee has met expectations. If you click "No," you can change it to "Yes" later if the employee meets the expectations.
Don’t click “Send for Approval” until all 90 days are complete. You can finish this earlier if the employee has met all the expectations and there are no more pending milestones to review.
📤 Sharing & Saving the Form
- The form is primarily for your use, but you can share it with your employee if helpful.
- Save a PDF copy at any time to capture a snapshot of progress.
- Upload the PDF to the employee’s folder or attach it to a note for documentation.
